Child asthma review (up to 12 years old)

Use this service to submit a routine review of a child’s asthma.

If you are older than 12 you will need to complete the Adult asthma review form.

You can use this service if:

• you are registered at the surgery
• you have been invited to do so
• the patient is age 4 to 11

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
